Effectiveness of bronchoscopic Examination In Children With Prolonged Cough

Abstract

Algorithm in diagnosis in the form of detailed questioning of anamnesis, therapeutic and diagnostic bronchoscopy in protracted and chronic cough, has shown the effectiveness of bronchoscopy to detect purulent endobronchitis, foreign bodies and congenital anomalies of the lower respiratory tract in children.The aim of the study is to optimize the methods of diagnosis and treatment of children with prolonged and chronic cough to prevent remote complications.

Materials and Methods
107 children with prolonged and chronic cough, previously repeatedly treated in different medical and preventive institutions, treated in 2017-23 were studied. A detailed collection of anamnesis, clinical and laboratory investigations, chest radiography, bronchoscopy were performed. Additionally on indications CT, MRI and study of external respiratory function.

Results
Age distribution: 2-4 years - 37 (34.6%), 5-10 years - 41 (38.3%), 11-15 years - 29 (27.1%). The rural population was 65 (60.7%) and urban population was 47 (39.3%). Boys - 59 (55.1%), girls - 48 (44.9%). They were admitted with diagnoses: chronic pneumonia - 22 (20.6%), chronic bronchitis - 36 (33.6%), congenital lung anomaly - 12 (11.2%), bronchial asthma - 17 (15.9%), lung atelectasis - 11 (10.3%), foreign body - in 9 (3.4%) patients. 39 (36.4%) had concomitant diseases: chronic sinusitis, anemia, biliary dyskinesia. The duration of the disease was from 1 to 6 years. Bronchoscopy revealed: purulent-fibrinous endobronchitis - 69 (64,48%), foreign bodies of respiratory tract - 28 (26,16%), bronchial adenoma and tracheobronchmalacia - 4 (3,73%), bronchiectasis in 6 (5,60%) patients.

Conclusion
Patients with prolonged or chronic cough, in case of ineffective treatment should be carefully collected anamnesis, examined by pulmonologist, allergologist, immunologist, ENT-doctors, pediatric surgeons to exclude purulent-inflammatory diseases and foreign bodies of the respiratory tract. In case of prolonged presence of foreign bodies in the bronchi and with purulent-fibrinous endobronchitis, it is necessary to repeat treatment and sanation bronchoscopy and dispensary-rehabilitation observation.

Biography

Mykyev Kalybek  graduated from the Kyrgyz State Medical Institute in 1974 with a degree in Pediatrics. He has over 50 years of professional experience as a pediatric surgeon and has participated in numerous international scientific studies in the fields of general pediatric and thoracic surgery. Since 1992, he has been engaged in parallel teaching activities at higher medical institutions, including the Kyrgyz State Medical Асademy named after I.K. Akhunbaev. Since 2009, he has been an associate professor at the Department of Pediatrics and Medical Genetics at the Kyrgyz-Russian Slavic University named after B.N. Yeltsin, and since 2014, he has been the head of the Department of Pediatric Surgery. He is the author of over 210 scientific publications, including 1 monograph, 1 textbook titled "Pediatric Surgery", 6 study guides, and 21 methodological recommendations for medical universities. He is currently finalizing a monograph titled "Frequently Ill Children from the Perspective of a Pediatric Surgeon."
